Charges may be incurred for appointments cancelled less than 24 hours before scheduled appointment time.

-It is your responsibility to let us know all insurance information, and of any insurance coverage changes. If you do not notify us of any changes, you will be responsible for all charges that may occur. 

-Due to the increased and changing health insurance provider participation fee schedules, all patients with high deductibles are subject to a down payment, in addition to any copays or outstanding patient balances.
